Privacy Policy

How Elite Spine & Pain Institute collects, uses, and protects patient and website-visitor information.

About this policy

This Privacy Policy explains how Elite California Pain and Spine Institute, doing business as Shamloo Elite Spine & Pain Institute (“the practice,” “we,” “us”), handles information collected through this website. It applies to website visitors and to anyone who contacts us through the site.

It does not replace our HIPAA Notice of Privacy Practices, which describes how we use and disclose the health information we create or receive as your health care provider. See “Your health information” below.

Effective date: September 15, 2026.

Information you give us

When you use the contact form, we receive the details you enter:

— Your name
— Your phone number
— Your email address
— Your message about how we can help

The contact form is for appointment requests and general questions. It is not a secure channel — please do not include detailed medical, insurance, or other sensitive personal information in your message.

Information collected automatically

Like most websites, the services that deliver this site receive standard technical information when you visit, such as your IP address, browser and device type, the page requested, and the date and time of the request.

The contact form is protected by Cloudflare Turnstile, which checks technical signals from your browser to tell people apart from automated spam.

We use Cloudflare Web Analytics to understand how the website is used in aggregate — for example, which pages are visited, which site referred you, your browser and device type, your approximate country, and how quickly pages load. It does not use cookies, does not store information on your device, and does not track you across other websites.

This website does not use advertising cookies or tracking pixels, and we do not sell or share your information for targeted advertising.

How we use information

We use the information described above to:

— Respond to your inquiry and arrange appointments
— Operate, secure, and improve this website
— Protect the website and contact form against spam and abuse
— Meet our legal and regulatory obligations

Service providers & third parties

We use a small number of service providers to run this website. Each receives only the information it needs to perform its service:

— Cloudflare — website hosting, security, cookieless website analytics, and contact-form spam protection
— Our form-processing and email systems — delivering contact-form messages to our office
— Google — web fonts, the embedded maps on location pages, and embedded YouTube videos

When your browser loads content from these services, they may receive your IP address and browser information under their own privacy policies. We do not sell personal information.

Your health information

Once you become a patient, the health information we create and maintain about you is protected health information under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law.

How we use and disclose that information, and your rights over it — including the right to see and obtain a copy of your records, to request corrections, and to receive an accounting of certain disclosures — are described in our Notice of Privacy Practices. You can download it from our Patient Resources page (assets/notice-of-privacy-practices.pdf), and a copy is available from our office on request.

Security & retention

This website is served over an encrypted (HTTPS) connection, and we use reasonable administrative, technical, and physical safeguards to protect the information we receive. No method of transmission over the internet is completely secure, which is why we ask you not to send sensitive medical information through the contact form.

We keep contact-form messages only as long as we need them to respond to you and to meet our legal and record-keeping obligations.

Your choices & rights

You may ask us what information we hold from your website inquiries, ask us to correct it, or ask us to delete it where we are not required to keep it. Depending on where you live, you may have additional rights under state law or, for patients of our Dubai practice, under the laws of the United Arab Emirates.

To make a request, call our office at 310-246-2358.

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from children through it. If you believe a child has submitted information through this website, please contact us and we will delete it.

Changes & contact

We may update this policy from time to time. The effective date above shows when it was last revised.
